Ready to enroll your child? Simply complete the following forms:
- Registration Form
- Tuition Contract
- Child Personal Health History
- Photo Release Form

Once you have completed the required forms, please submit your registration fee of $75.00. Your enrollment paperwork will not be processed until your registration fee is received. Your options for payment include:
- Online using Give Central (found under the Make a Payment link)
- Check (please make checks payable to The Church of Holy Apostles). Checks can be mailed or dropped off at the parish or preschool office.
- Cash (please bring to the parish or preschool office)
